Overview for Active Participants

Eligibility

Active employees must have contributed $1,375 per month to be eligible for coverage. Eligibility will be month to month. A Dollar Bank for your coverage will grow to a maximum of six months of coverage ($8,250). Once that is full, everything over $1,375 per month goes into your HRA to be used for deductibles, coinsurance, dental, vision, other medical payments, and self-payments.

Hearing
Benefits include $2,000 once every five years for hearing aids or repairs (batteries are not included). For dependents under age 16, the Fund covers a replacement hearing aid annually.

Vision

Benefits include $500 every two years for exams, frames, lenses, and contact lenses. (The allowance is replenished on the first day of every even year.)

In addition, the Plan pays 100% (up to $500 per eye, once per lifetime) for LASIK eye surgery.

Death Benefit
In the event of your death, your beneficiary will receive $8,000.
Accident & Sickness Weekly Benefit
For a non-occupational injury or sickness, you will receive $300 per week up to 26 weeks. This benefit applies only if you are not an owner or officer of the employer and you are not receiving accident or sick pay from your employer.
Accidental Death Benefit
If your death occurs due to an accident, your beneficiary will receive $8,000. (This is the principal sum.) If the accident results in the loss of a limb, you will receive a percentage of the principal sum.
