Annual Out-of-Pocket Maximum
This is the most you could pay out of pocket each year for prescription drugs.
$2,500 per person (an additional $1,800 per person if specialty drugs prescribed)
$5,900 per family (an additional $1,800 per family if specialty drugs prescribed)

80% paid by Plan
Birth Control Pills
Plan pays 100% for all FDA-approved contraceptive methods and counseling (includes barrier and hormonal methods and implanted devices)
Erectile Dysfunction Medication
50% paid by Plan
(up to 6 pills per month)
GLP-1 Medication
80% paid by Plan if taking to treat diabetes
50% paid by Plan if taking for weight management, coinsurance does not count toward out-of-pocket maximum and must work with a TEAM wellness and nutrition professional
